School Social Worker - Chicago Public Schools

Under the supervision of personnel from the Office of Diverse Learner Supports and Services and in collaboration with the school principal, School Social Workers are responsible for the educational, psychological, and social development, progress, tracking, assessment and support of all students in assigned classes/caseloads, and performs other duties as assigned and required. School social workers are assigned to schools based on the citywide needs of the district of which requirements related to general and special education services, evaluation, staffing, and mandated counseling are carried out in conjunction with the educational, psychological, and health disciplines.

Specialty Functions:

- A valid license issued by the Illinois State Board of Education, endorsed as a School Social Worker

- Provide counseling services to students in both general and special education programs, using individual, group and classroom methodologies, along with consult services to faculty, co-team members, parents, and other appropriate parties regarding socio-cultural and emotional factors impacting the student’s learning experiences

- Perform diagnostic assessments for students needing initial evaluations, re-evaluations or special evaluations and participate in all meetings/conferences to determine eligibility for and adequacy of services on an annual or as-needed basis


Minimum Qualifications & Requirements:

- A graduate degree in social work from from a program accredited by the Council on Social Work Education (CSWE)

-Completion of the CPS online application and teaching profile at cps.edu/teach

-A valid license or credential issued by the state licensing board, with the appropriate endorsement(s) for the role

Employment Expectations:

As a condition of employment with the Chicago Public Schools (CPS), an employee must be an actual resident of the City of Chicago or be willing to become a resident within six months of the date of his/her employment with Chicago Public Schools. For certain high-needs positions, exemptions to this policy may be available.

Job Responsibilities

Job Responsibilities

MINIMUM EDUCATION AND EXPERIENCE:

Licensed Clinical Social Worker (LCSW), prior experience as a field instructor, experience in social work education or on a university campus is highly desirable.

KNOWLEDGE REQUIREMENTS:
• Strong written and oral communication skills, as well as excellent organizational and interpersonal skills.
• This position will be located on the Urbana-Champaign campus.
• Travel required, primarily within the State of Illinois.

Regional Case Manager - Maximus


Maximus is hiring a Remote Regional Case Manager, based in Illinois, providing a support function for a new contract for the State of Illinois where we will be performing mental health assessments for individuals that will be admitted to nursing facilities, specialized living programs, and specialized mental health residential facilities. We help determine proper placement, that mental health support services are identified and provided, and the individual is transitioned back to the community when appropriate with the proper support for their needs.

Requirements:

Field Social Worker / Social Worker IPU Part-time / Weekend Social Worker @ Lightways Hospice and Serious Illness Care


Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children.

The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 20-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.

We currently have the following Social Worker positions open:

Field Social Worker – In this full time Field position, the Social Worker provides short-term crisis intervention and individual or family counseling and support as determined by the social worker’s assessment, family input, and the IDT plan of care. Also develops and facilitates relationships with referral sources and provides information to patients and families/caregivers as appropriate. Must have availability to do on-call as needed.

Position requires a licensed Social Worker with Masters degree and LSW required with a minimum of one year experience in either a hospital setting, home health or nursing home/long term care. Hospice experience preferred.

Social Worker IPU Part-time - This position works in our inpatient unit focusing on our patients and families psychosocial needs. The position works Monday, Tuesday and Wednesday of week 1 and Thursday, Friday of week 2. The Social Worker assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Also develops and facilitates relationships with referral sources and provides information to patients and families/caregivers as appropriate.

Position requires a licensed Social Worker with Masters degree required and a minimum of one year experience in either a hospital setting, home health or nursing home/long term care. Hospice experience preferred.

Weekend Social Worker - This position works throughout our service area attending to our patients and families psychosocial needs. The position works 12 hour shifts every Saturday, Sunday and one other day (Friday or Monday preferred) for full time. The Social Worker provides short-term crisis intervention and individual or family counseling and support as determined by the social worker’s assessment, family input, and the IDT plan of care. Also develops and facilitates relationships with referral sources and provides information to patients and families/caregivers as appropriate. Must have availability to do on-call as needed.

Position requires a licensed Social Worker with Masters degree required and a minimum of one year experience in either a hospital setting, home health or nursing home/long term care. Hospice experience preferred.

The clinician’s role includes screening for diagnosis or suspicion of serious mental illness and or intellectual disability, and medical necessity for short or long-term nursing facility placement.


Primary Responsibilities:

• Responsible for pre-admission and concurrent reviews for admissions and continued placement in Medicaid certified nursing facilities, utilizing federal and State’s criteria
• Review and communicate all information with providers, utilizing computer database system for determining nursing facility placement appropriateness
• Utilize both referral screens and medical records review to make determinations
• Develop and document a clinical rationale supporting the review decision as the final step in the review process


Minimum Requirements:

• Graduation from an accredited school of nursing, with current RN, LPN, MSW, or related license, in good standing, as dictated by the client contract.
• Minimum of two (2) years of experience working with individuals with a mental health diagnosis and/or intellectual disability, with the primary focus on behavioral health, as dictated by the client contract
• Minimum of two (2) years of medical experience with a strong understanding of rehabilitation potential, based on diagnosis, age, disease process and chronicity also as dictated by the client contract
• Perform all job duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies and procedures
• Excellent knowledge of diverse cultures and issues, with the ability to incorporate special needs into utilization review decisions
• Working knowledge and understanding of behavioral health diagnoses, psychotropic medications, medical diagnoses and prescribed medications
• Excellent ability to multitask while effectively performing job duties, ensuring all voice mail and email messages are monitored and responded to within 2 hours of receipt
• Excellent written and verbal communication skills
• Ability to interact professionally with providers (hospital & nursing home staff, physicians, community service workers, etc.), state contract officers, and coworkers while seeking or providing information
• Ability to collect data, define problems, establish facts, and draw valid conclusions for supporting a superior written rationale for review determination
• Must work within a tight turnaround time assuring contract compliance
• Excellent computer skills with experience in word processing, email, and spreadsheets (Microsoft Word, Outlook and Excel preferred)
